About This Report

The Persistency Report summarizes categories of HCC data for reported chronic conditions that should be reported annually. The report identifies whether there is a lack of persistence in HCC reporting for chronic conditions (for instance - traditional timing issues of encounters for members not being managed by their primary care provider).

Persistency %Ratio of recurring HCCs to all HCCs.
RAF Score Request IDRAF Calculation ID the report uses.
CMS Plan IDContract number for each plan.
Member IDVoyager ID.
Part AB Total ScoreRisk score for Medicare A/B -- does not include Part D.
Part AB Total Pre-FFS Scoree Risk score for Medicare A/B, pre-fee-for-service program benefits.
Part AB Demo ScoreDemographic-only risk score -- for Medicare A/B does not include Part D or diagnosis score.
Part AB Interaction ScoreDiagnosis interaction risk score for Medicare A/B -- does not include Part D.
RAF Type CodeRAF type assigned to member (for instance, CN for Community Non-Dual).
Status1 = Active, 0 = Inactive
HCCHierarchical condition category
ScoreHCC coefficient.
PersistThe persist-relevant HCC code.
Persist Count1 = Yes, it persisted. 0 = No, it did not persist.
Persist Score1 = HCC coefficient recorded; Yes, it persisted. 0 = No, it did not persist.
